On the paperwork checklist ALL applicants must provide us with:

1. R-229 - Green & White Form
     Questions 1 through 16 must be completed along with:
- Signature of test taker in the proper place
- Signature, CT License # and expiration date of parent
        (home training instructor)
This form can be completed on paperwork turn-in day or
            picked up and completed prior to paperwork day                        

2.  Parental Consent Form (2D) -  must be properly completed.
            Can be filled out on paperwork turn-in day or picked up
            And completed prior to paperwork day

       If you attempt to complete either of these forms ahead of time you MUST complete them properly.  Use the links below for instructions for proper completion.

3.  Valid Permit - submit a copy of your picture permit on paperwork
      Turn-in day and you must bring your permit the day of the test

4. CS1 - small yellow certificate shows completion of classroom
             work.  HRDS full teen students are issued this after turning in
             their paperwork, all other students must provide this.                          

If you have taken the test already you must also provide:

A $40 check made out to the DMV    
                                                           
CS1  it is given to you after your test. 
                                                          
R-279  This is a pink sheet that is given to you by the DMV inspector.

The information below is provided for individuals who are planning on taking their driver's test at our location.  Proceed if this applies to you. 

ONLY 16 or 17 year olds With a picture permit can take their driving test at a driving school.  You must come with a parent or legal guardian to complete the paperwork.  Your parent or legal guardian must bring their valid Connecticut Driver's License.

Please follow ALL the steps to ensure you provide us with all the information required by the DMV (failure to provide all the proper information may result in you not being able to test when you desire).
Step 1.  If you have not done so already, please visit our test registration page by clicking here, and download the proper form and mail it to us with your registration fee.
Step 3 Bring all your documents to the High Ridge Driving School at your                                                     assigned time - to check your date and time click here 
               If this time does not work for you give us a call and we can schedule another time.
Our Next Available Test Date is December 16, 2009
  You MUST turn in a COMPLETE test application
on Thursday, November 24th between 3:00p & 5:00p.
